Ⅰ. 서론(Introduction)
조직공학은 유익한 플랫폼 개발을 추진하기 위해 세포 및 분자 생물학과 유전학, 생화학, 바이오의학 공학의 첨단 기술을 통합한다. 인공기관/기기의 이식이 최종적으로 성공하는데 필요한 요소는 숙주가 제품을 면역학적으로 수용하는데 있기 때문에, 체내 면역 계통이 이식 장기/기기를 거부하는 사례를 완화하는 것은 조직공학 분야의 주요 과제로 남아 있다.
Ⅱ. 본론(Main Subject)
줄기세포 응용을 통한 첨단 조직공학은 손상 조직의 재생에 유망한 전략으로 보인다. 그러나 이식 세포는 저산소율이나 인공 스캐폴드와 선천 조직의 상호작용 저하, 영양소 부족과 같은 주요 장벽을 만나게 된다. 이에 따라 줄기세포는 이식 전에 사전에 조절할 필요가 있다. 줄기세포의 사전 또는 사후 조절에는 아래와 같이 다양한 기법이 사용된다: 저산소 처리 및 줄기세포 노화.
소염반응이나 이주, 혈관생성 과정과 같은 속성의 유전자 변형은 줄기세포의 안정성 제고에 사용되었다. 이식 부위의 염증 반응은 줄기세포의 생존율에 영향을 미치기 때문에 표적 조직에서 줄기세포의 효능은 소염 반응 저하로 인해 크게 약화되었다. 이식 이 주 후 종양괴사인자수용체(TNFR)를 이용한 MSCs-TNFR의 변형은 TNF-α, IL-1β, 및 IL-6과 같은 염증 사이토카인과 단백질의 양을 저하시켰다. 이식 줄기세포에서 분비되는 성장 인자인 VEGF 및 bFGF의 높은 분비율은 혈관형성 작용을 촉진하여 심근 혈관 기질을 구축하고 혈관 기능을 강화하였다.
줄기세포에 약물을 첨가하면 체내 줄기세포의 안정성을 강화할 수 있는 것으로 보인다. 콜레스테롤 생합성 억제제인 스타틴은 줄기세포의 조절률과 내피기능을 강화하였다. 심바스타틴과 로바스타틴은 PI3K/AKT나 ERK1/2와 같은 2개 조절 경로를 억제하는데 활용되어 긍정적인 줄기세포 안정성 강화를 달성하였다.
자가 소경 혈관은 혈관 질환 치료에 이식되었으나 혈관 조직 공학 분야는 환자에게 제공할 수 있는 인공 조직 공급에 제약이 있다. 다형핵백혈구(L-lactic acid)-co-poly (epsilon-caprolactone) (P(LLA-CL 70:30)), 다형핵백혈구 코팅(dopamine) polycaprolactone (PCL), 다형핵백혈구(lactic-co-glycolic acid) (PLGA)와 같은 합성 중합체를 이용하여 조작한 인공 혈관은 손상된 혈관의 치환을 위한 대체 수단으로 간주되었다. 그러나 전기방사법을 이용하여 조작한 인공 혈관의 실제 사용은 순간 혈전증이나 이식 후 비후와 같은 부작용으로 인해 여전히 단점을 안고 있다.
줄기세포 치료는 줄기세포나 간세포 분화를 통해 생산되는 신형 세포로 사멸 세포를 대체하여 MI 치료에 적합한 수단을 대표할 수 있다. ST-분절-상승 심근경색(MI)이나 비-ST-분절 상승 MI, 스텐트 이식을 이용한 심장혈관형성술, 관동맥 우회로 이식술(CABG), 항혈전 요법과 같은 바이오의학 기법은 MI 병변 치료에 빈번하게 사용되었다. 그러나 이 기법은 심근세포를 새로 재생하지 않기 때문에 괴사세포가 근본적으로 치환되지 않을 경우 MI는 악화될 수 있다. 이에 따라 줄기세포는 MI의 증상을 완화하여 스텐트 유래 재협착증을 완화하는 첨단 방식으로 간주할 수 있다.
심혈관 조직 재생에 적용되는 세포치료 방식은 여러 가지 한계를 안고 있다. 관상동맥 협착이나 차단으로 인해 관상동맥을 통한 세포 투여가 제약을 겪을 수 있다. 줄기세포를 MI 부위에 투여하는 데는 두 가지 접근법이 있다: (1) 혈관을 통해 (2) 심근에 직접 세포를 주사한다. 전신 기관이나 조직에 세포를 정맥내 주입하는 것은 임상적으로 제한되었기 때문에 체내에 주입해야 하는 세포수는 대규모가 되어야 한다.
히드로겔 주입은 수술 없이 줄기세포를 MI 병변에 투여하는 최신 방식으로 간주되었다. 주입된 고분자 히드로겔은 MI 병변의 확장 및 경직에 영향을 미칠 수 있기 때문에 주입된 세포는 MI 병변을 둘러싼 저산소 증상과 같은 열악한 환경에서 약화된다. 구획을 제거한 장기 ECM은 장기 구조와 3차원 구조를 흡사하게 시뮬레이션하는 성장인자를 함유하기 때문에 체내 줄기세포의 생존율을 강화하는 최신 바이오원료 스캐폴드로 대두되었다. 구획을 제거한 돼지 심근 조직을 쥐의 MI 병변에 주입한 결과 겔화 스캐폴드가 형성되어 상당한 부위 및 체내 세포부착을 달성한 것으로 보고되었다.
줄기세포뿐 아니라 간세포는 패치를 통해 심장 내 MI 병변에 투여할 수 있다. 패치는 생체적합률 증가와 분해성 억제 및 유연성과 같이 여러 유익한 속성을 갖고 있다. 그러나 수술이나 줄기세포 효능의 모호성과 같은 단점도 안고 있다
각종 생체적합 히드로겔과 결합한 나노섬유는 효능 강화를 위해 도입되었다. 예를 들어 Poly (glycerol sebacate) (PGS)로 제조된 단편 나노섬유는 core (PGS)-shell (PLLA) 전기방사로 제조되었다. 헥산과 DCM (1:2) 배합물에 나노섬유를 침지하여 PLLA (Shell)을 제거한 후 단편 PGS 나노섬유를 도출하였다. 악티닌이나 트로포닌, 미오신 중연쇄, 코넥신 43과 같은 각종 심장 표지는 단편 나노섬유에서 배양된 심장근육세포에서 과잉 발현되었다. PGS는 주사제라서 심근경색 치료에 최소 침습 기법이 사용된다.
Ⅲ . 결론 및 시사점(conclusion)
줄기세포를 함유한 나노섬유는 면역거부를 완화한 조직 재생의 새 시대를 열었다. 다양한 나노섬유 변형을 도입하여 줄기세포에 가장 효과적인 운반체로 나노섬유를 최적화하였다. 줄기세포가 적재된 나노섬유는 죽상동맥경화증이나 심근재생을 비롯한 심혈관 질환의 치료에 활용될 수 있는 것으로 입증되었다.참고: 본 논문은 필자의 이전 논문 '검토: 심혈관 조직 공학용 나노섬유'에서 일부 업데이트되었다. Expert Opinion on Drug Delivery, 10(11):1565-1582 (2013).
